  1. Critically discuss the concept of patient activation with reference to relevant research and policy. You are required to illustrate potential outcomes of patient activation with examples from your field of practice and placement, and/or experiences shared by PCVs (Patients and Carers Voices) during various workshops throughout the module. (25%/750 words) 2. Critically explore challenges to caring for transgender patients and identify potential ways to address these in practice, in the context of health inequalities and discrimination. You are required to include relevant evidence-based literature, key legislation and policies, along with examples from your clinical practice or examples from literature in your field of practice to support your arguments.
