Indicators of challenges in transitioning

  Within any population or setting, list and discuss at least 6 indicators that a patient and/or their family may have difficulty or challenges transitioning to the next level of care. (2-3 pages) (home>hospital; hospital>home; home>assisted living, etc). It could be a patient you've seen or from course readings indicators that you would have assessed as a potential problem.
