These case plans should be individualized and tailored to the needs of the offender based on discussions between these professionals.
The main components of a case plan typically include an assessment of the offender’s risk factors, such as their probability of recidivism; any treatment needs that must be addressed; goals which are specific objectives set out by the professional team; interventions which are strategies or activities used to meet those goals; resources available that can help the offender reach those goals (such as therapy sessions or job training); monitoring measures put in place by the professionals involved with managing the case plan; and timelines for expected progress along each stage included in the plan.
