  Using the direction on creating a survey in Survey Monkey, Create a 15 question Agency Assessment Tool with responses in the form of a numerical Likert Scale similar to what was learned in HUS 311. The survey should include the Categories separated by at least four headers used in the Quantitative Social Research Course. Pay special attention to the type of questions for each category/header.
