Care of a person approaching end of life

The purpose of the assessment is for you to critically reflect upon your role in the care of a person approaching the end of life (someone who has withdrawn from hemodialysis). You should incorporate an analysis of the changing patterns of death and dying relevant to your patient who has stopped dialysis, discussing how these have influenced trajectories of decline for people living with life-limiting illnesses. You will identify relevant assessments to address potential unmet needs, communication strategies that facilitate achieving person-centered goals of care and interventions that support the person’s physical, mental and spiritual wellbeing. Your assignment should demonstrate the integration of scholarly literature and evidence of engagement with recommended resources.
