Communication defines the key components of bedside nursing practice. Patient safety is a national and global concern in all healthcare institutions. In hospital setting, patient safety is the precedence in endruing care therefore, communication slip-ups are the collective cause of adverse events during patient care. Unfortunately, poor communication results in 60% of adverse events in healthcare settings (WHO, 2018). With several factors that contributes to communication breakdown during bedside handoff that potentiates to adverse events such as inaccuracy of treatment and medical errors in hospitals (Muller, et al., 2018). Thus, making it a priority to create a structured communication that can improve current nursing practice in all healthcare. The change I propose is establishing a proper handoff during shift change. Currently, there has been a lot of issues found in my current unit where mistakes or incorrect report have been given because nurses forget to relay information which have led to errors that can easily be prevented with proper bedside handoff.
